Service Description

This service allows the issuance or renewal of health certificates

Eligibility or Criteria

The applicant must be an employee in the establishment requiring the certificate

Required Documents

Original passport and a copy

Copy of the establishment health license

Copy of the signature authorization of the establishment owner

One (1) personal photo for the applicant

Civil ID and a copy, for Kuwaiti citizens and GCC (Gulf Cooperation Council) citizens

Procedures

Visit the Municipality governorate branch concerned with the service - Permits for Municipal Services Department - Permits for Municipal Services Inspection - Health Certificate Section and submit the required documents

Submit a request to the employee in charge at the section concerned with the service for checking and approval

Pay the fees due at the Municipality cashier's counter (Stamps)

Fill the required medical check-up application form through the employee of the section concerned

Have the medical check-up. After finishing the check-up and obtaining the physical fitness result through the Ministry of Health, the health certificate record will be renewed and approved by the employee in charge at the section concerned
