This service allows you to obtain an examination and treatment request for the emergency cases in the Accident Units

Applicant or his/her representative must attend in person -

Visit, the applicant patient or his/her representative, the Medical Records Office of the Accident Unit to obtain the examination and treatment request
Refer to the Accident Unit concerned

 Variable Fees
(0) KD Fixed Fees: For Kuwaiti citizens
(1) KD Fixed Fees: Stamp, in case a transfer document from the Health Center the patient is transferred from is available, for non-Kuwaiti citizens
(2) KD Fixed Fees: Stamp, in case no transfer document from the Health Center the patient is transferred from is available, for non-Kuwaiti citizens
(5) KD Fixed Fees: Stamp, for visitors
