Transferring Patients to the Outpatient Clinics in the Public Hospitals For the First Time  

 
Service Description
This service allows the patient citizens and residents to open a medical file and, hence, to be transferred to the outpatient clinics in the public hospitals -

Eligibility or Criteria
Applicant patient must be transferred with a specified date from the health center to the specialized clinic in the hospital -

Required Documents
Civil ID -  
Patient transfer document from the health center to the outpatient clinic -  
Health insurance card, if the applicant is a non-Kuwaiti citizen -

Procedures
Visit, the applicant patient, the Medical Records Section in the Outpatient Clinic on the specified date to open the medical file
Present your personal identification. Then, you, the applicant patient, will be transferred to clinic concerned
