Service Description
This service allows the issuance of a medical report on the condition of the patient based on his medical record in the hospital
 -

Eligibility or Criteria
The applicant patient must attend in person, if he is alive -  
A first-degree relative of the patient must apply in person, if the patient is alive but cannot attend in person -  
The applicant must refer to the Legal Affairs Department of the Ministry of Health, if the patient concerned is deceased -  
The request must be submitted through the Prison Authorities, if the applicant is imprisoned -  
The applicant must refer to the Legal Affairs Department of the Ministry of Health, if he has a pending investigation in a legal case -  
The applicant must have a medical record in the hospital -

Visit the Office of Medical Reports and Commissions in the hospital administration concerned and submit the required documents
