Vaccine card issued from the Preventive Health Center the child refers to, for children -  
International vaccine card (if available), for adults -

Procedures
Visit the nearest Preventive Health Center depending on the place of residence and submit the required documents, in case the applicant is a child. If the applicant is an adult, visit the nearest Preventive Health Center depending on the place of residence
