Service Description
This service allows citizens, residents, and visitors to issue a true copy from the birth certificate, as a replacement for lost or damaged certificate, to present it to the educational entities. -

Eligibility or Criteria
The applicant or his agent must attend in person -

Required Documents
Applicant's personal identification (civil ID, nationality certificate, or passport) -  
Copy from the birth certificate, if any. In case no copy from birth certificate is available, the birth date and location, and the birth certificate issuing center must be known -

Visit the Central Record for Birth and Death Department of the Ministry of Health and submit the required documents
