Service Description

  • This service allows citizens, residents, and visitors to issue a true copy from the birth certificate, as a replacement for lost or damaged certificate, to present it to the educational entities.

Eligibility or Criteria

  • The applicant or his agent must attend in person

Required Documents

  • Applicant's personal identification (civil ID, nationality certificate, or passport)
  • Copy from the birth certificate, if any. In case no copy from birth certificate is available, the birth date and location, and the birth certificate issuing center must be known

Procedures

Visit the Central Record for Birth and Death Department of the Ministry of Health and submit the required documents
Pay the fees due
