Issuing the Portfolio with the Personal Radiation Measurement Card

Service Description

  • This service allows citizens and residents working in the field of using ionic radiation to measure the radiation exposure in the medical, industrial and research applications

Eligibility or Criteria

  • The applicant must submit an official letter requesting to measure the radiation exposure

Required Documents

  • Copy of the personal license to work in the radiation field
  • Official letter requesting to measure the radiation
  • Copy of the civil ID
  • Copy of the employment request
  • Copy of the fee payment receipt, for employees of the non-government entities only
  • Copy of the certificate of the radiation dose exposed to during the previous working periods outside the State of Kuwait
  • List of employees in the radiation field stating the employee name and work

Procedures

Visit the Department of Calibration and Radiation Exposure Measure of the Ministry of Health, and submit the required documents. Submit the list of employees in the radiation field stating the employee name and work
Fill the following service application forms: form to issue personal license to work in the ionic radiation field, radiation measurement portfolio request form, declaration and undertaking on committing to the technical terms of using the portfolio and paying its value in case of loss or damage
Pay the fees due
Receive the application form and attachments, and register it with an import number before the 18th of the month. Receive the Personal Radiation Measurement Portfolio for the new month after delivering the previous month portfolio. Afterwards, the cards are prepared to measure and evaluate the radiation doses according to the approved technical proceedings in this concern. Then, the radiation dose the applicant has been exposed to is measured and saved using a software to save the data and a monthly report is printed for the doses. This monthly report for the measured radiation doses and notes, if any, is sent to the applicant entity
In case another system to measure the radiation doses is applied by a third entity, the licensed entity must regularly send the radiation dose report to the Department of Calibration and Radiation Exposure Measure of the Ministry of Health. In case radiation doses above the applicable levels are detected, the entity is notified, and coordination is made to know the reasons for such over-doses. A report on this matter is prepared and presented to the Radiation Protection Department manager to take the necessary proceedings
