Service Description

  • This service allows citizens and residents to request the convening of a medical committee to examine the patient health condition

Eligibility or Criteria

  • Applicant must have a medical record in the hospital

Required Documents

  • Applicant's civil ID
  • Letter from the applicant's entity requesting to convene the committee (on receiving treatment abroad - occupational health - medical council)
  • Treating doctor's report

Procedures

Visit the Office of Medical Reports and Commissions in the hospital administration concerned and submit the required documents
Fill the service application form
Pay the fees due. A date will be set for the committee to convene, and then the patient will be presented before the committee in the specified date. Afterwards, the report will be sent to the applicant's entity
