Service Description

  • This service allows you to obtain an examination and treatment request for the emergency cases in the Accident Units

Eligibility or Criteria

  • Applicant or his/her representative must attend in person

Required Documents

  • Civil ID
  • Transfer document from the Health Center
  • Health insurance card, for residents

Procedures

Visit, the applicant patient or his/her representative, the Medical Records Office of the Accident Unit to obtain the examination and treatment request
Refer to the Accident Unit concerned

Fees

(0) KD
 Variable Fees
(0) KD Fixed Fees: For Kuwaiti citizens
(1) KD Fixed Fees: Stamp, in case a transfer document from the Health Center the patient is transferred from is available, for non-Kuwaiti citizens
(2) KD Fixed Fees: Stamp, in case no transfer document from the Health Center the patient is transferred from is available, for non-Kuwaiti citizens
(5) KD Fixed Fees: Stamp, for visitors
